K-12 Faith Formation Registration

K-12 Faith Formation Registration for Catechetical Year 2025-2026

(Must submit separate form for each child)

Faith Formation and Sacrament Preparation for Children & Youth 2025-2026 (October-April)

It is through catechesis, (Religious Education) that we continue to grow in our Catholic faith and to allow our children to broaden their faith and knowledge of Jesus. We welcome your child(ren), whether he or she is preparing to receive a sacrament or enrolling in on-going catechesis.

Must submit separate form for each child. Registration form, payment, and copies of sacraments required prior to confirmation of enrollment.
